Physicals: Any student participating in a sport must have a current physical on file. Please check the expiration date of their last physical on file at the school by checking their student account online in PowerSchool.
If you need a sports physical, Northwest Sports & Exercise Medicine is offering two physical nights exclusive to Gonzaga Prep students. They are Monday, August 18, 2025 from 5:30-7:30pm (Last name A-M 5:30-6:30 and N-Z 6:30-7:30), and Wednesday, August 20, 2025, from 6:00 - 8:00 pm (Last name A-M 6:00-7:00 and N-Z 7:00-8:00). Cost is $30.

ATHLETIC FORMS - FORM INSTRUCTION GUIDE
In addition to a physical exam, the following forms must be completed and on file in the Athletic office. PLEASE NOTE: Forms for the 2024-2025 school year will be available in August through the School Information System - PowerSchool.
- Physical Form: A sports physical from a physician is required every two years. This must be filled out by an approved medical professional and a copy is to be kept on file at the school. The Gonzaga Prep downloadable sports physical form can be found at the bottom of this page.
- Concussion Management Online Agreement One form each year to be submitted and digitally signed by student and parent.
- Sudden Cardiac Arrest Awareness Agreement One form each year to be submitted and digitally signed by student and parent.
- Emergency Information Form - Online Submission One form each year to be submitted and digitally signed by student and parent.
- Athletic Code Online Agreement One form each year to be submitted digitally signed by student and parent.
* This form is only necessary if your child is driving themselves, or riding with another student to games or practices.
